Do I Need a Doctor's Requisition?

Yes. More than 440 of the 445 tests AvoVita offers can be ordered without a physician requisition. You choose the test, book the appointment, and the results come to you.

The exceptions

Five tests require a physician requisition to be present at the time of collection:

• EPISEEK Early Cancer Detection — Precision Epigenomics
• Harmony® Non-Invasive Prenatal Test (NIPT) — Dynacare
• MaterniT® Genome Expanded NIPT — Dynacare
• MaterniT® 21 PLUS NIPT — Dynacare

Each provides a downloadable requisition form on its test page for your physician to complete. Without it, the specimen cannot be collected.

Why these tests are different

Each reports directly to a referring physician rather than to you. That is a condition set by the performing laboratory, not by AvoVita.

Every other test in the catalogue can be ordered directly, including hormone panels, thyroid testing, vitamin and mineral testing, cardiovascular markers, heavy metals and infectious disease screening.
